University Health Report During COVID-19

We encourage you to submit your health report, as soon as possible. We understand that obtaining a signed health report may be more difficult given the current COVID-19 pandemic. Please see our page on the University Health Report to download a copy of the health report today.

Health reports should be submitted via email 617-373-2601 (fax) or email No paper reports will be accepted, given the current COVID19 pandemic. 

Students are required to be fully COVID-19 vaccinated before the start of the fall 2021 term and will need to submit documentation of their vaccination through a separate process that will be announced in due course.   Please monitor your Northeastern email for more information about how to submit your COVID-19 vaccination information.

University Health and Counseling Services remains committed to caring for students at Northeastern and welcoming our newest Huskies. The University Health Report is a requirement for all incoming students. We understand that obtaining a signed health report may be more difficult given the current COVID-19 pandemic. If you encounter difficulties there are a few options available for you and your family to consider:

  • Obtaining your vaccination record through current provider – We recommend reaching out to your primary care office as soon as possible. Given that many offices have shifted the normal operations, there may be a delay in obtaining this information. If there is a delay or any barrier to receiving this information, we ask that you submit the incomplete health report with any information that you do have.  
  • Vaccinations – If you are aware of any vaccinations that you have not had, you do not have record of receiving, or you were unable to access in your home country, you may reach out to your primary care provider to inquire about options. You can also contact a local walk-in clinic or pharmacy to see if they offer the vaccinations you need.
  • Titer testing – If you are unsure about vaccinations or immunizations and want to be tested, you may request a laboratory titer test from your primary care physician or walk-in clinic.

Insurance Coverage

Prior to any vaccinations or titer testing, we recommend contacting your insurance company regarding the price and coverage. The cost associated may vary depending upon location, test, and vaccine.

UPDATE – I am on the Boston campus, am I required to get a flu shot?

In accordance with the MA Department of Public Health announcement (1/15/2021), the flu vaccination is no longer required for students. However getting your flu vaccine is still strongly encouraged as we protect from both the flu and it could decrease symptoms that could be confused with COVID-19. Is there a deadline for submitting my health report? Has the deadline been extended due to the pandemic?

The deadline for submitting your health report is June 30, 2021. The health report requires your complete demographic information, consent to treat at UHCS, emergency information, and the second page that needs to be completed by a health care provider.

I am unable to get my doctor’s office to fill out the vaccination section. What should I do?

We understand it may be difficult to obtain certain information. However, it is important that you submit your health report with any information you currently have. We recommend calling your primary care physician’s office, as soon as possible, and asking what your options are for obtaining this information. As a result of COVID-19, many offices have shifted their operations and there may a delay in receiving this documentation. Your primary care office should complete page two of the University Health Report, or submit a signed list of vaccinations and immunizations.

Some health care offices provide an online portal where patients can access their medical information. If you physician is unable to complete the health form requirements and you have access to an online health portal associated with your medical care provider, you can download or print a copy of your immunization information. You can submit this information along with your health report form.

I am unable to make an appointment at my doctor’s office to receive vaccines that I am missing. What should I do?

If you are unable to receive vaccines from your primary care physician, we recommend that you contact a local pharmacy or walk-in clinic for information about vaccine administration and availability. Prior to receiving any vaccinations or titer testing, call your health insurance provider using the number on the back of your insurance card to discuss cost. Vaccinations may be subject to different pricing at different locations depending on your insurance.

Can I get a titer blood test in place of vaccination requirements?

You may have a blood test called an antibody titer that shows whether you are already immune to measles, mumps, rubella, varicella, and Hepatitis B. You will still need to get a tetanus/diphtheria/pertussis (Tdap) vaccine and/or Meningitis vaccine if they are out of date. The antibody titer tests can be done at your local health care provider office.

Please note, if your titer results are either negative or equivocal, you may need another immunization to boost your immunity. Please review this result with your clinician.

What will happen if I get a vaccine and it is past the date it was supposed to be given?

In order for immunizations to be effective, they must be given on a schedule. It is okay to receive vaccines later than scheduled, but not earlier than scheduled. The vaccination will still be effective once the series is complete. If you are unable to get a vaccination during the time frame that was recommended, it is important you get the vaccine as soon as possible.

How should I submit my health report?

Please do not mail your completed University Health Report form to the office or attempt to drop it off in person. Instead, please return it to UHCS in one of the following ways*: 

*While we make every attempt to keep your emails private, please note that email is not considered a confidential form of communication since it could be intercepted in transmission or misdirected (see Confidentiality and Email section on our Confidentiality page). For this reason, faxing your form is the preferred method of submitting reports.

Due to the COVID-19 pandemic, I am not able to complete a series of vaccines that I am needing. Should I hold off on submitting the University Health Report until I do so?

The deadline to submit your health report is June 30, 2020. Please submit your health report with all vaccinations documented. If you are missing any vaccinations, please attempt to get the missing vaccinations before arriving to campus. Some vaccination series take longer than the time that is available to complete.  In this case, we ask that you get the first vaccine in the series and submit the corresponding documentation.

Will I be able to receive needed vaccines at UHCS in the fall?

UHCS typically provides Shots and Immunizations as well as titer testing. Please call us to discuss your vaccination needs 617.373.2772.

Massachusetts Immunization Information System (MIIS)   Massachusetts law (M.G.L. c. 111, Section 24M) requires providers to report immunization information to a computerized immunization registry known as the Massachusetts Immunization Information System (MIIS). 

  • The MIIS stores immunization records for you and your clinician and can help prevent outbreaks of disease like measles and the flu. 
  • All information in the MIIS is kept secure and confidential. 
  • The MIIS allows information to be shared with health care providers, boards of public health and state agencies concerned with immunization. 
  • You have the right to object to the sharing of your immunization information across providers in the MIIS. If you object, please complete this form and bring it to your appointment or submit it at
