DS-2019 Application Process

Step 1: Set up myNEU Credentials

Please begin by going to, and click the link entitled “How do I get a myNEU
username and password?”

On the next screen, click the “Activate Account” link found next to the word “Students.”

After reading the text on the next screen, click the “Accept” button.

On the next screen, named “Account Activation,” you will need to fill in some information to activate your myNEU account. When you are asked to enter your 5 digit zip code, please enter 02115. You will also be prompted to enter the last four digits of your International ID. You will have received this information from your Northeastern University department. If you have lost this information, please contact your department to retrieve this information.

After completing all the information on the “Account Activation” page your myNEU account will be created. Please note! It may take up to 24 hours until your myOGS account is then created.

Step 2: Request DS-2019 from myOGS System

To access the e-form, begin by clicking on the following link:

You will next be taken to a screen where you will be prompted to enter your myNEU credentials. If you
happen to have forgotten your myNEU password, click the link on this screen to begin re-accessing or resetting
that information.

After you log on – you will be taken directly to your DS-2019 Request E-Form

In order to complete the E-Form, you will need to upload a color JPG or PDF image of the identity page of your passport. This is the page that contains your picture, as well as information such as your name and date of birth.

You will also need to upload the necessary financial documents to show that you have the funds to meet your expenses while at Northeastern University. This document must be in English. If you are submitting a bank statement, please know that it must be less than 9 months old at the time you will begin your studies at Northeastern. For example, if you are due to begin at Northeastern on January 8, 2018, then any financial statement you upload must be dated form April 8, 2017 or later.

If you have a sponsor who will support you financially (this can even be a family member), then you must attach a letter of support, in English, from your sponsor. You can find a sample letter on the university website:

When you are done filling out the e-form, make sure to hit the Submit button at the bottom of the online form. If you are in the middle of filling out the e-form and realize you cannot complete it in one sitting, and need to return, you can save your place by scrolling to the bottom and clicking on the Save Draft button.

When you have successfully submitted your e-form, you will receive an automated email receipt in your student email account. The email will be sent from

The email will have a subject line of “receipt: DS-2019 Request Received.” At this time the University will review your request, and contact you by email in case there are any questions about your request.

If you realize after submitting your request that you have made a mistake, or if the University contacts you and informs you that your request cannot be approved due to an error, then you can Cancel your request and begin again.

To do this, click on the same link to access the e-form, and log in with your MyNEU username and password. You will now see the e-form you submitted. At this time, scroll to the bottom of the e-form and click the Cancel button. This will re-set the form to a blank state and you can begin again.

What is DS-2019?

As an exchange visitor in J1 non-immigrant visa category for individuals that have been approved to participate in the U.S. in exchange visitor program, you will need a Certificate of Eligibility called DS-2019, which you will use to apply for J1 visa at your home country.

How long will it take for me to receive my DS-2019?

If you will provide sufficient documentations in timely manner, then you should be able to get your completed DS-2019 document in two-week time frame.

Will I need another DS-2019, if my spouse or child are accompanying me?

Yes, you will. In this case, you will need to apply for additional DS-2019s (one for each dependent). You will also be expected to provide additional documentation for each dependent with your request.

What documents do I need to upload for obtaining DS-2019?

You will need the color copy of passport, affidavit of support with recent copy of financial document, showing the evidence of liquid asset – all uploaded on the e-form.

What is School Code on my DS-2019?

It is on your DS-2019 and it is P-1-03057.

Do I need to have different DS-2019, if I am non-degree exchange visitor?

The document of DS-2019 is the same, and it does not look different, except the section on major area of study.

How will I know if I am subject to two-year home residency requirement on my DS-2019?

You will see it marked on the bottom of the document, if you are subject.

Will my DS-2019 need to be signed before I travel to US?

Yes, you will have to sign your DS-2019 on the bottom of the document and the name of the city, where you will be studying, too.
