Commencement and Student Record Name Change:
Complete this form and return it with the applicable documentation if you would like to change your name on your graduation application and diploma.
Your request must be processed by the Registrar’s Office no less than 6 weeks prior to the ceremony to ensure that your diploma will be printed correctly.
If you are changing your name 6 weeks after you’ve received your diploma, you will need to fill out the Replacement Diploma Form instead.
If you need a diploma notarized please visit the Office of the Registrar’s website.
If you need more information about Apostilles, please visit the Secretary of the Commonwealth’s website.
The process will take 4-6 weeks. During our peak time (March through May) replacement diplomas will take 8-10 weeks to process and we are unable to rush diplomas during this time.
There is a fee of $50 for each copy
PLEASE NOTE: Only students are authorized to order a replacement for themselves. Others are not permitted to order on behalf of another student.
If you have not received your original copy of your diploma, DO NOT FILL OUT THIS FORM. Please email us at firstname.lastname@example.org with your name, NUID, and graduation date so we can investigate the status of your diploma.
