License Agreement
To submit your license agreement online for take these steps:
1. Read through all five pages of the License Agreement.
2. At the bottom of page five, complete the acceptance form.
3. Submit the form and print out a copy of the receipt for your files
4. Please pay close attention to the cancellation dates.
