Course Number Registry Forms
Course Number Registry forms are used to establish new courses and to initiate changes to existing courses. The forms are in Microsoft Word format and can be dowloaded using the links below.
After a course receives formal approval at the college level, the Course Number Registry Form is submitted electronically as directed on the form.
