Open Enrollment begins Monday 11/21/11 and ends Friday 12/2/11
The Open Enrollment period is your annual chance to enroll and/or make changes to your medical, dental, medical reimbursement account and dependent reimbursement account elections. You may also enroll in the optional life insurance program during this period.
Your medical and dental coverage will remain the same if you do not make any changes.
You must, however, re-enroll in the medical reimbursement account and dependent reimbursement account for 2012 if you wish to participate. To enroll for the 2012 plan year, please complete a Benefits Enrollment Form (PDF).

Benefits staff will be available at the following locations during the open enrollment period:
Completed enrollment forms may be sent to HRM/Benefits through interoffice mail to 250CP, dropped off at 250 Columbus Place or faxed to ext. 7610.
The last day to submit benefit enrollment forms is December 2, 2011.
