Massachusetts Health Care Reform

Effective July 1, 2007, the Massachusetts Health Care Reform law requires all Massachusetts residents who are 18 or older to have health insurance. If you decide to waive health insurance coverage, you will be required to complete a Commonwealth of Massachusetts Health Insurance Responsibility Disclosure Form (HIRD) (PDF) as part of the enrollment process. This form requests you to indicate if you have alternative health insurance coverage and also requests your acknowledgement of the requirements to have medical coverage and the penalties for non-compliance. You will be asked to complete a new HIRD form each plan year.

Each year, by January 31, the University’s health insurance carrier will send a 1099-HC form to each plan subscriber who resides in Massachusetts. You and your dependents (if applicable) will need this form to file your state income tax return.
