Life Insurance

Northeastern provides a comprehensive life-insurance program to benefits-eligible faculty and staff that includes basic life insurance for employees as well as optional life insurance that employees may purchase for themselves and/or their eligible dependents. In addition, the life-insurance program provides valuable added features and services such as portability, guarantee issue, and survivor support. The life insurance program is insured by Liberty Mutual.

Basic Life Insurance
The Basic Life Insurance Plan provides two times annual base salary, to a maximum of $500,000. This coverage is fully paid by the university. Enrollment is automatic, so it is important to designate a beneficiary. (age reduction schedule applies after age 65)

Imputed Income
Under the current tax laws, the value of basic life insurance that exceeds $50,000 is subject to federal, Social Security and state taxes, if applicable. The taxable value of the life insurance, which the IRS calls imputed income, will appear on your W-2 at the end of the year.

Optional Life Insurance
The Optional Life Insurance Program provides the opportunity to purchase, through convenient after-tax payroll deduction, optional life insurance for employees and/or their eligible dependents. The three optional life insurance plans are Supplemental Employee, Spouse/Same-Sex Spousal Equivalent and Child(ren) Insurance.

Supplemental Employee Life Insurance

The Supplemental Life Insurance Program provides the opportunity to purchase additional life insurance above the two times base salary provided by Northeastern. You can purchase an additional 1x, 2x, 3x, or 4x your base salary to a maximum of $500,000 (age reduction schedule applies after age 65). Your total life insurance coverage, basic and supplemental combined, will be limited to $1 million.

During the guarantee issue period you may select 1x or 2x base salary without a Statement of Health. Please refer to the age rate table (on right) for the Supplemental Life Rates.

If you are eligible and would like to enroll complete a Benefits Enrollment Form (PDF) and return it to HRM/Benefits at 250 Columbus Place. If you are selecting any amount above 2x base salary, you must also complete the online Statement of Health with Liberty Mutual for approval. Spouse/Same-Sex Spousal Equivalent Life Insurance

The Optional Spouse/Same Sex Spousal Equivalent Insurance will allow you to purchase between $25,000 and $100,000 in life insurance for your spouse/same-sex spousal equivalent. Elections can be made in increments of $25,000 and are limited to 100 percent of your basic and supplemental life insurance combined.

During the guarantee issue period you may select $25,000 without a Statement of Health. Any amount above $25,000 will require evidence of insurability. The rates for Optional Spouse/Same-Sex Spousal Equivalent Insurance are based on the spouse's age, and follow the same rate schedule as the employee Supplemental Life Plan. Please refer to the age rated table for Supplemental Life Rates.

If you are eligible and would like to enroll, complete a Benefits Enrollment Form (PDF) and return it to HRM/Benefits at 250 Columbus Place. If you are selecting any amount above $25,000, you must also complete the online Statement of Health with Liberty Mutual for approval. Age Reduction Schedule for Optional Life Insurance

At age 65, the value of your insurance policy will be reduced to 65 percent.

At age 70, the value of your insurance policy will be reduced to 50 percent.

Child(ren) Life Insurance

The Optional Child Life Insurance provides the opportunity to purchase $10,000 or $20,000 for each dependent child under age 26. Purchase of $10,000 or $20,000 can be made without a Statement of Health. The rates for the Optional Child Life Insurance are $1 per $10,000 per month. Please note separate elections for each child is not necessary; one election will cover all eligible children. Benefits for infants under 6 months are limited to $1,000 in coverage.

If you are eligible and would like to enroll, complete a Benefits Enrollment Form (PDF) and return it to HRM/Benefits at 250 Columbus Place. Please note, if an employee's child(ren) no longer meet the eligibility guidelines, it is the responsibility of the employee to terminate the policy by contacting a member of the Benefits department.

Guarantee Issue
The optional life insurance program has a feature called guarantee issue. This is the amount of life insurance that can be purchased without providing a Statement of Health. The Statement of Health is required by Liberty Mutual to verify good health and is in the form of a online questionnaire. The Guarantee Issue Period is during the 30 days following your initial eligibility (i.e. date of hire) or change-in-family status, which includes birth, adoption and marriage. Coverages elected after the Guarantee Issue Period must be done during open enrollment and the Statement of Health will be required.

Age Rated Table
The rates for both the Supplemental Employee and Optional Spouse/Same-sex Spousal Equivalent Insurance are based on an age rated schedule and coverage amount. Supplemental Employee Life is based on the employee age and the Optional Spouse/Same-Sex Spousal Equivalent Insurance is based on the spouse/same-sex spousal equivalent age. The rates will increase the first of each year following a salary increase and/or movement to a different age band. The rates below are guaranteed until January 2015.
