Google Forms

Google Forms is a simple Google Docs tool that you can use to create an online form or survey. Form input is instantly added to a Google spreadsheet that you can view at any time. After you create a form you can email it to others or embed the form directly in a webpage. You can share the spreadsheet that contains the form data with other Google Docs users.
