Traumatic or Acquired Brain Injury

Each student interested in seeking services through the DRC should complete an Intake Packet. Once we have a complete Intake Packet from you, we will begin our review process, which may take 2-3 weeks.

An Intake Packet consists of all of the following items:

  1. Completed Student Disclosure Form
  2. Diagnostic Documentation including a complete and current Neuropsych Report.
         NOTE: You may also choose to include copies of any IEP and/or 504 Plan as supplementary information. However, these pieces alone do not constitute adequate documentation at the post-secondary level.
  3. Transfer students: If you received services/accommodations at another institution, please have the Disability Services office fax a verification letter indicating the accommodations you were approved to use. (Confidential fax: (617) 373-7800)
Once the Intake Packet information is complete, and reviewed, and if documentation is determined to be acceptable for accommodations and services, you will be asked to meet with a Specialist to complete the Registration Process.
