Hearing Loss

Each student interested in seeking services through the DRC should complete an Intake Packet. Once we have a complete Intake Packet from you, we will begin our review process, which may take 2-3 weeks.

An Intake Packet consists of all of the following items:

  1. Completed Student Disclosure Form
  2. Diagnostic Documentation consisting of a complete Disability Disclosure Form and Audiological Evaluation completed within the past three (3) years.
         NOTE: If the hearing impairment is likely to fluctuate, this information will need to be updated annually by an appropriate licensed professional.
  3. Transfer students: If you received services/accommodations at another institution, please have the Disability Services office fax a verification letter indicating the accommodations you were approved to use. (Confidential fax: (617) 373-7800)
Once the Intake Packet information is complete, and reviewed, and if documentation is determined to be acceptable for accommodations and services, you will be asked to meet with a Specialist to complete the Registration Process.
