AD(H)D

Each student interested in seeking services through the DRC should complete an Intake Packet. Once we have a complete Intake Packet from you, we will begin our review process, which may take 2-3 weeks.

An Intake Packet Consists of all of the following items:

  1. Completed Student Disclosure Form
  2. Appropriate documentation reflects a student's historical and current level of functioning, describes what substantial impact(s)/limitation(s) the student experiences due to the disability, and includes a diagnostic statement. Documentation may consist of a combination of the following:
    • Neuropsych Report
    • Psycho-Educational Evaluation
    • Copy of IEP(s)
    • Copy of 504 Plan(s)
    • Copy of Summary of Performance (SOP)
  3. Transfer students: If you received services/accommodations at another institution, please have the Disability Services office fax a verification letter indicating the accommodations you were approved to use. (Confidential fax: (617) 373-7800)
Once the Intake Packet information is complete, and reviewed, and if documentation is determined to be acceptable for accommodations and services, you will be asked to meet with a Specialist to complete the Registration Process.
