A FACT report/case (e.g. “a FACT”) is a notification and record of faculty concerns or updates of a concern about a student’s progress, performance, behavior, and/or an issue the student has reported. Accessed via the “FACT Photo Roster” link in every course, instructors can enter information about a particular issue, the steps they plan to take with the student, the steps they suggest for the student, and actions they’d like an advisor to take. Unlike email, FACT reports are stored in an enduring system that is accessible to all relevant staff (if for instance a student’s primary advisor is unavailable) and also makes it easy to track trends in individual student performance, as well as help programs determine needs for supplemental support and communication.
