Are FACT reports required for all students?

Faculty are encouraged to submit a FACT case for any student in her/his course whose academic performance or behavior in their class will negatively affect their academic progress, and are expected to submit for any student who they suspect may have had an Academic Integrity Lapse such as plagiarism or unauthorized collaboration.
