For time sensitive FACT cases, a faculty is requested to submit reports or updates 1-2 times depending on the length of the course. However, instructors may submit updates throughout the term if there are multiple changes in the student’s progress or performance.
FACT reports do not need to be submitted for minor updates to an originally reported issue, but should be submitted if a student’s performance subsequently worsens or improves.
