Online application

Create an ApplyYourself account, complete and submit your application, then come back and review your progress. Instructions and tips are available on the ApplyYourself site.

PlusOne Admission Application

This form must be completed by current NU undergraduate students who are applying to a BA/BS-MA/MPA degree program. Please refer to page 2 for departments that offer PlusOne degree programs and department contact information. Please be aware that the deadline is August 1st.

Internal Admission Application Notification Form

This form must be completed by current students who wish to apply to a degree program from Special Student to Master’s or PhD, Master’s to PhD or one Master’s program to another within the same department.

Leave of Absence

Requests for non-medical LOA's should be made to departments prior to the start of the semester for which the leave is being requested. Please note that the form and instructions are two pages. This form is appropriate for all non-medical leaves.

Transfer Credit Form

This form must be completed by students who wish to transfer credits from another US accredited institution or from the College of Professional Studies.

Directed Study and Readings Courses Form

Students wanting to enroll in Directed Study or Readings courses must complete the Directed Study and Readings Contract prior to the start of semester in which they wish to enroll. Specifics such as reading lists, reading assignments and deliverables as well as the grading method must be provided by the instructor. Upon approval, registration in the course must take place prior to the end of the second week of the semester.
