List of what to do to finish your co-op (Click on link for more detail)

Please note: You will not receive credit for co-op if you do not submit EVERYTHING as outlined below.
Here is what you MUST turn in (by the deadline) to receive a SATISFACTORY grade for your co-op
MANDATORY
This deadline is the 15th of the month AFTER you complete the co-op (i.e. Jan 15th, or July 15th) (If the 15th falls on a weekend – everything is due the following Monday)
#1 Complete the student self-evaluation (on my NEUCOOL) (if you do this 1st – it can help you write the paper)
#2 Complete this reflection paper (guidelines are spelled out in the "list" at the link above)
#3 Get a signed Evaluation completed by your employers *(all employers have been asked to do this online now - but there are hard copies - on this page - in case they really don't want to do it on-line)
#4 Turn all of this in to me before the deadline to make sure I have everything I need to give you a grade.

#5 I am trying to improve the co-op experience (and positions) for the Sociology, Human Services, and Cultural Anthropology majors. I would appreciate if you would give me feedback on the process, jobs, working with me, etc. (it is completely anonymous, so please be honest). A LINK TO THIS SURVEY IS BELOW
#6 Rate your job and what you liked and didn't like in COOL < go to your placements and click on “rate for students”> (this helps us evaluate the site you worked at - is it a good job?, are they helping you as much as they tell us they will, etc.) When students look at the jobs in MyNEU COOL, this can help them "feel it out more"

Performance Evaluation
The employers are being asked to answer these questions on-line. Normally you would give this form to your employer at the end of your co-op. You should still remind them of it and ask to meet with you to discuss the results. (Some students and employers may also use this on their first day of work to set goals for the Co-op as well) (This is provided in both MS-Word and Adobe Acrobat formats.)
(once employers have filled this out - the results will be in COOL. You will be able to see their feedback once you have completed the self-evaluation)
